Creative Lab Tile Installation Testimonial

1. Each customer is only allowed for ONE submission only.

2. Customers need to fill up all compulsory fields and all details and particulars submitted must be accurate, true and complete. Incomplete, inaccurate or illegible submission form will be disqualified.

3. Customers who are eligible will be entitled to RM10 Touch 'n Go credit. Limited to first 10 customers only.

4. Submission is only valid on Creative Lab's website page shared by Creative Lab Sales Designers. Any other form of submissions shall not be entertained.

5. The Touch 'n Go reload pin will be sent to customer’s email subject to The Company’s verification based on details submitted within 1 month after verification.

6. The Touch 'n Go reload pin is not transferable, exchangeable for cash or any items.

7. Customer will not be able to do any changes once the form is submitted.

8. All participants must agree to the Terms and Conditions stated.

9. Submission of this survey form shall constitute consent from customers to allow the use of customer’s name, photo, image, voice and/or likeness by Niro Ceramic Creative Lab Sdn. Bhd. for editorial, advertising, promotional, marketing and/or other purposes without further compensation except where prohibited by law.

10. The Company has the rights to substitute the prize or product with the same value item.

11. The Company reserve the rights to change/amend any of the mechanics without prior notice.
